Our forms are available to you in two formats: Microsoft Word 2000 or in Acrobat 5.0. Please let us know if you have difficulties.

| You can download forms in Microsoft Word. You may be prompted
for a password when you attempt to open them. Simply click "cancel" and the form
will open. You can solve this problem by down-loading an upgrade to Word 2000 from www.microsoft.com. You can also open the forms in Word
95, although some of the form fields may collapse to a smaller size than in the original
layout.
Once you’ve opened the Word form, either save it to your hard drive or print it out. If you save it, you can fill it out electronically and email the completed document to us. If you print it out, you can complete the form by hand, then scan and email or fax it to us. |

You can download forms in Acrobat, and open the forms in Acrobat Reader to print them out. Acrobat Reader is available free at www.acrobat.com. Complete the form by hand, then scan and email or fax it to us. |
